Documents for the first appointment

Please take a copy of the following documents with you to your first appointment:

Laboratory findings
Current medical reports
Computed tomography CT
Magnetic resonance imaging (MRI) or PET findings
or at most endoscopy findings

In the case of imaging examinations such as CT or MRI, please also take the CD of the images with you.

To speed up the availability of these findings and imaging, it is helpful if you send us written consent in advance that we can also request such examination findings from institutes and hospitals.
